ISMP Safe Medication 2011 Assessment

The Institute for Safe Medication Practices 2011 Medication Safety Self Assessment® is available online for hospitals to evaluate their medication safety practices, identify opportunities for improvement and compare experiences with demographically similar organizations.

In 1999, the Institute for Safe Medication Practices and the American Hospital Association formed a partnership to improve medication safety in U.S. hospitals. In 2000, 2004 and 2011, ISMP, AHA and HRET distributed the ISMP Medication Safety Self Assessment® to all US hospitals.

Findings from the 2000 and 2004 assessments showed opportunities for system improvements to reduce the risk of medication errors. Hospitals that completed the self-assessment used the findings to establish and implement plans to improve medication safety systems. ISMP, AHA, and HRET used the findings to design and disseminate three comprehensive and diverse Pathways for Medication Safety™ tools to help hospital leaders and clinicians reduce the risk of medication errors.

The 2011 assessment documented the progress of U.S. hospitals in medication safety and identified the impact of such emerging challenges as staffing shortages, shrinking reimbursement and new technology. This project was funded by the Commonwealth Fund.
